Please enter all information in full; our office needs the information requested in order to provide identifiers to your insurance that allow us to check on your insurance coverage and benefits. This is to determine the least expensive method of sessions for you. Once we receive your information, we will contact you to discuss your benefits and coverage, and schedule an initial voice evaluation appointment. Clients with Insurance: Referrals and Prior Authorizations can be faxed to 505-501-7483 or emailed to: info@gendervoicecenter.org.
Private Pay: $40 per 30 minute voice training session. The initial voice assessment is a flat one-time fee of $100.
Clients are required to have a credit card on file with us prior to the first session using our secure, encrypted, HIPAA-compliant system SimplePractice. Credit cards will be charged for any copay, co-insurance, balance billing, private pay, and/or late cancel/no show fee after each session. It is fine for someone else to place their credit card on file for you. Medicaid and Tricare clients may not be required to have a credit card on file.
Sessions are typically weekly. Every other week sessions may be available upon request and based on availability. Sessions can also be more than once a week.
